Job Summary: The Family Support Coordinator plays a vital role in supporting families with children aged birth to age 5 facing complex needs. The role involves collaborating with Family Support Coordinators to ensure efficient implementation of the Family Support Coordination strategy. The incumbent will be responsible for recruiting families, facilitating the intake and enrollment process, developing service plans, providing resources and referrals, monitoring progress, and supporting continuous quality improvement. Essential Functions: Essential functions, as defined under the Americans with Disabilities Act, may include, but are not limited to, the following tasks, duties, knowledge, skills and other characteristics. ESSENTIAL DUTIES AND RESPONSIBILITIES generally include the following. Other duties may be assigned:
Actively recruit new families for enrollment in Family Support Coordination Services through community engagement and distribution of information. Quarterly networking with other providers to share information and support referrals. Intake & enrollment:Contact families after receiving a referral to complete the Protective Factors Supports Pre-Survey. Discuss the program's purpose, expectations, and assess if Family Support Coordination is suitable based on the family's needs. Schedule meetings with families for enrollment and service planning Maintain waitlist of eligible families interested in services. Service Planning:Develop written service plans with individualized goals, objectives, steps, and timeframes. Maintain confidential files for each family, documenting interactions, Protective Factors surveys, and service plans. Resources & Referrals to Services:Provide families with specific information and referrals to support services based on their service plan goals. Obtain written consent for sharing information with other service providers. Provide families with relevant documentation for navigating community services. Progress Monitoring:Follow up with families at least once per week to check on service connections and address barriers. Conduct monthly meetings with families to review service plans and evaluate progress. Transition Planning and Disenrollment:Collaborate with families to determine appropriate transition from services. Administer the Protective Factors Concrete Supports Post Survey upon program exit. Provide families with relevant documentation for addressing potential needs post-service. Continuous Quality Improvement:Implement a process of continuous self-monitoring, reflection, and quality improvement. Collect satisfaction ratings from disenrolled families and review data to enhance program practices. Creates a family-centered environment that provides helpful and accurate information and referral services to families with children ages birth to 5. Maintains liaisons with community and social service agencies. Through researching and contacting social service agencies, assists in obtaining information to continually update available resources. Maintains data for Family Resource Center Coordinator's reports. May assist with evening and Saturday adult/child interactions or outreach, as needed. Performs other duties as assigned or required. KNOWLEDGE AND SKILLS: Knowledge of applicable Federal, state, county and city statutes, rules, policies and procedures Knowledge of applicable Peoria Unified School District and departmental policies and procedures Knowledge of functions of available community resources. Skill in providing oral and written communication. Ability to establish and maintain effective working relations with co-workers, students, parents, and the general public in a professional manner. Skill in operating a personal computer utilizing a variety of software applications. Well-developed observational and record keeping skills. Excellent organizational skills. Ability to perform a variety of duties in a fast-paced environment. Ability to work independently Ability to organize own time in an effort to meet assigned situations and work demands MINIMUM QUALIFICATIONS:Must have a minimum of a bachelor's degree in social work, early childhood development, early childhood education, family studies, or a related field.
Have at least one year of experience in providing services for high risk/high needs populations. Are knowledgeable about resources available to families in the community and how to connect them to supports and services to address needs. Are familiar with the First Things First Family Support Coordination Standards of Practice and contract. Receive initial professional development on: culturally responsive service delivery reflective of the community and target population (i.e. race, ethnicity, class, gender, local community, religion, and family structures, implicit bias, etc.) and the Protective Factors Framework. Receive at least eighteen hours of professional development annually that is relevant to the target population/community/service delivery. Professional development may be provided through formal training or informally within the organization. The minimum caseload per full time equivalent staff is 10, not to exceed 25, and is determined based on families' needs. OTHER REQUIREMENTS: Must be able to pass a fingerprint and background clearance check. Must be able to obtain Fingerprint Clearance Card May be required to lift and/or carry materials and supplies weighing up to 25 pounds. Must possess and maintain a valid Arizona driver's license. May be required to travel to perform work functions. May be required to work outside normal working hours. All staff are expected to meet the qualifications for their position. If programs are seeking to hire staff that do not, a Hiring Request must be submitted to First Things First for approval prior to
