The D irector of Strategic Community Partnerships is responsible for leading Presbyterian Health Plan's (PHP) efforts to foster robust relationships with community-based organizations , faith organizations, local governmental agencies , and key national groups for the purpose of addressing social determinants that affect the health and wellbeing of PHP members and communities. Partnerships will be prioritized based on findings of community health assessments and member needs, and input from key stakeholders, including PHP clinical operations and The Center for Community Health. Core responsibilities include:
Representing PHP in local and statewide forums and leading regionally-based staff in building local partnerships. Coordinates with operational leads across PHP to align priorities, implement partnerships and evaluate outcomes. Leverages data and analytics, and considers social determinants of health, to identify innovative community investments and partnerships and develops the business case for adoption and implementation. Lead work teams through definition, design, and creation of specific program project plans, and creates and manages timelines, budgets, completes project status reports, facilitates team meetings, and maintains progress through completion. Bachelors degree required. 5 years relevant experience required. 3+ years leadership experience.  Health Plan experience preferred.  Demonstrated success bringing together disparate stakeholders to work toward a shared vision. Knowledge of the unique strengths and needs of New Mexico communities. Strong and adaptable communicator able to convey a shared vision to a variety of audiences, including internal leaders, community service providers, and members. Ability to harness common purpose into well-defined partnerships, programs, and interventions with demonstrated outcomes. Statewide travel required Foster robust and effective partnerships with community-based organizations, faith organizations, local governmental agencies, and key national groups for the purpose of addressing social determinants that affect the health and wellbeing of PHP members and communities. Lead a cyclical process based on data and input from key stakeholders to understand social needs and health disparities across the state and identify capacity building and key partnership opportunities to address needs/disparities. Create opportunities for regular and robust dialogue with community partners, including shared visioning, priority and goal settings, and delineation of shared responsibilities, including CBO regional councils to coordinate efforts and topical summits on subpopulation needs/targeted health disparities. Develop and execute a process for targeting and providing multi-model investment and support for community-based partners, including evaluation and monitoring of the impact of investments on member and community outcomes and health disparities. Develop programs/partnerships with key partners that address needs of special and/or vulnerable populations. Collaborate with AO/technology stakeholders to achieve meaningful bidirectional data exchange with community partners, including closed-loop referrals with community-based organizations. Provide oversight to member-facing teams working to address social needs in the community.

The compensation range for this role takes into account a wide range of factors, including but not limited to experience and training, internal equity, and other business and organizational needs.
