RN Director of Case Management Fort Myers, Ohio The RN Director of Case Management has overall responsibility for managing and coordinating department activities. The RN Director of Case Management ensures staff compliance with organizational policies and external regulatory agencies. The RN Director of Case Management coordinates the integration of the department's patient care and discharge planning processes with related hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization. The RN Director of Case Management provides oversight for all aspects of daily hospital Case Management operations and is accountable for achieving established outcomes through actively engaging interdisciplinary teams and external stakeholders.
Qualifications: Registered Nurse required Bachelor's Degree in Nursing required Master's Degree preferred Director of Case Management experience in an acute hospital setting Responsibilities: Provides leadership for the assessment, development, implementation and evaluation of safe, cost-effective care delivered within the Case Management department Maintains knowledge/ensures compliance of and serves as resource for hospital policies and procedures, professional practice standards and regulatory body requirements Collaborates, communicates and maintains effective relationships with staff, physicians, other healthcare providers and colleagues, and the community Assumes accountability for the cost-effective financial performance for all resources within the area(s) of responsibility Prepares, monitors and responds to variances in the operating and capital budgets Performs needs assessments and provides/coordinates educational activities to provide for the learning needs of staff Utilizes data to analyze, develop and implement quality improvement and patient satisfaction initiatives that support a safe environment, regulatory expectations, reduction of risk and positive clinical outcomes for targeted patient populations Develops/assists in the review and revision of hospital/division/unit policies, procedures and protocols Integrates best practices in the delivery of Case Management services Assumes responsibility and accountability for individual knowledge, skills, performance and behavior in accordance with hospital standards of care and policies and procedures Supports the mission, values, and vision of the organization For our Case Management opportunities, feel free to forward a resume to Michelle Boeckmann at or visit our Case Management website at
