Full job description Introduction The Office of the Secretary (OS) within the Department of Health Services (DHS), is recruiting to fill a Data Collection and Forms Specialist position in the Communications Office.

You will have the option to be headquartered at the Madison, Green Bay, Milwaukee, or Waukesha regional office.
Position Summary DHS currently manages over 7,500 forms which are available in varying formats. As the Data Collection and Forms Specialist, you will be responsible for duties related to implementing data collection standards on behalf of the organization and ensuring forms meet DHS Digital Asset Guiding Principles. Responsibilities include:
Designing, reviewing, and maintaining forms used by the agency. Maintaining the agency forms management practices in compliance with laws, policies, and procedures. Analyzing and documenting customer business and data needs utilizing professional, department, and state standards to ensure efficient collection, retention, and retrieval of data for internal agency or external customer use. Enhancing accessibility functionality and framework of data collection and the protection of sensitive data. Providing technical assistance and guidance to others on all forms management activities and ensuring all forms adhere to DHS standards. Acting as back-up for DHS's form data collection survey software reviews and work closely with the enterprise records officer on records management activities. Salary Information Pay is between $26.00 and $29.12 per hour (approximately between $54,000 and $60,500 per year), depending on qualifications, plus excellent benefits. A 12-month probationary period is required.
This position is classified as Records/Forms Management Specialist and is in pay schedule and range 07/04. Pay for current or former employees is based on the rules that apply to compensation for the applicable transaction.
Job Details DHS strives to hire individuals who demonstrate the ability to communicate and effectively interact with people across cultures, ranges of ability, genders, ethnicities, and races. All applicants who may be appointed will be required to allow DHS to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled. Employees must have access to both a private workspace and secure high-speed internet to meet the needs of remote work. Mileage reimbursement to the employee's headquarter location is not provided. DHS does not sponsor work visas for this position, thus, in compliance with federal law, selected candidates will be required to verify eligibility to work in the United States by completing the required I-9 form upon hire. For positions that do allow remote working flexibility, although residency in the State of WI is not required for permanent positions, it is not anticipated that working outside of WI or bordering cities will be allowed as positions may at times need to report to office locations in WI on short notice. Qualifications Minimally qualified applicants will have all the following:
Experience creating fillable forms using a variety of software tools such as Microsoft (MS) Word, MS Excel, Adobe Acrobat, etc. Experience editing and proofreading content to ensure proper formatting, grammar, and workplace style. Experience uploading or publishing professional content on the internet or intranet such as forms, publications, policies, procedures, etc. Experience providing technical assistance or guidance to multiple levels of staff within an organization. Well-qualified applicants will also have one or more of the following:
Experience creating Americans with Disabilities Act (ADA) compliant accessible forms. Experience with workflow development in an HTML 5 environment. Advanced forms experience which includes using Microsoft Word content controls, Microsoft Excel formulas, or Adobe Acrobat tagging. The letter of qualifications is limited to (1) one page.
