Community Navigator - This is a grant-funded position through December 2024.
Trellis, a nonprofit organization, advocates for older adults. We provide community organizations with funding, and older adults with information and connections to access the best resources to help them age well. By partnering with community and healthcare organizations, we strive to build age- and dementia-friendly networks that make daily life easier for older adults. Provides targeted outreach and community-based education to older adults in the City of Minneapolis and helps navigate individuals to services and resources related to aging, including those provided by the Senior LinkAge Line and Trellis' Juniper and Dementia Friends, Pensions Departments, as well as city, county and state programs and beyond. Focuses on low-income and under-served populations who may face barriers to accessing services.
Qualifications: Minimum of two years related experience in social services, education, or street outreach setting required. Bilingual in English with Spanish, Afaan Oromo, Hmong, or other language preferred. Effective communication skills in both oral and written formats with individuals at all levels of the organization, with partner organizations and diverse community members. Demonstrated experience networking with older adults in diverse communities, including using flexible approaches to gain trust within the community and deliver results. Valid driver's license and proof of insurance required, or alternative transportation plan. Strong computer skills with experience in Microsoft Office Suite Able to work a flexible schedule, including evening and weekend hours. Travel / Driving-This position is community-based, and the individual is required to travel to various locations in the City of Minneapolis. They may be required to travel to 2 or 3 locations each day.
