Full job description SUMMARY OF PRIMARY FUNCTION: Primary functions of the Community Assistance Programs Coordinator are to assist households in locating and receiving community services available to them. Responsibilities include processing client requests for community assistance services, providing information and referrals to other programs. The Community Assistance Programs Coordinator has direct contact with households to include home visits and provides program support duties.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: Outreach to Youth, Individuals, Families and Communities Proactively establishes contacts and connections with public and private community agencies. Collects, organizes and distributes information and materials regarding Community Services programs to families and communities to enhance awareness of Community Services programs. Facilitate community and parent meetings in response to Community Services programs. Coordinates training offered by Community Services programs for parents and community members throughout the service area. Completes intake forms for various Community Services Programs. Program Management Establishes and maintains individual household files. Completes the household intake information. Discusses the household's history with them. With the Community Assistance Team, determines the most suitable program(s) to assist the household's needs. Enters household data into statewide data systems. Attends state and regional meetings as expected by program guidelines. Assures payments are paid on time. Maintains knowledge of the various program expectations, rules and work plans. Assists Community Services team with grant writing. Tracks financial records for community assistance programs. Case Management Assists the household with applications for support services and employment. Assesses total household needs, immediate and future. Discusses the household's goals, assists household in setting and meeting goals. Conducts home visits as requested. Visits with an adult household member as needed but at least monthly to assess the current status and determines if suggested referrals were helpful. Establishes a realistic housing plan with household members. Documents all contacts with household. Maintains adequate documentation on all instances of refusing assistance. Advocates for all individuals seeking assistance through Community Services. Acts as a liaison for individuals and families to community supports, resources and agencies when needed. Encourages individuals and family's members to move toward self-advocacy and help them develop self-advocacy skills Provide information and skill building on various topics to individuals and family members. Programmatic Duties Attends monthly staff briefings and other appropriate meetings and trainings with the Community Assistance Team, as required. Assists the team in gathering information for program reports. Submits reports as indicated by the agency policies and program guidelines. Manages HMIS reporting, data entry and training. Supervisor and Leadership Supervise staff as directed by Community Services Director. Ensures quality services are provided by their staff; provides comprehensive training on duties, responsibilities and expectations of the position; offering continued support, guidance and resources as needed to be successful. Monitors staff performance, providing positive and negative feedback in a timely manner. Conducts annual performance appraisals, supports staff in defining and meeting goals and professional development needs. Addresses performance concerns in a timely manner, holding staff accountable for their actions. Offers constructive feedback, clear expectations, and necessary support to promote a positive outcome. Seeks assistance from management and the Human Resources Director as needed to ensure staff are given sufficient support in meeting the expectations of their position. Communicates and enforces the expectation of a professional and positive work environment, treating everyone with respect. Leading by example, energizes and encourages staff to commit to excellence; empowering them to succeed, making each day a positive experience for themselves and others. Provides solid, unified, positive leadership for staff in times of challenge or change; displaying support and confidence, adapting priorities and focus as needed. Effectively plans short- and long-term goals for success; personally, and for the work group. Effectively manages multiple projects, commitments, and the work produced by staff; ensuring deadlines are met and quality of work meets expectations. Ensures a safe, positive and productive work environment is provided to staff. Community Action Outcome Measures Completes exit form(s) for Community Assistance programs. Completes follow up surveys for various Community Assistance programs. Enters the outcome data into the various data base systems in place. Associates degree or two years equivalent education and/or experience in social work, community services or related field. Two years' experience in a similar position, including functions and responsibilities related to social work, community services or related. One-year experience working directly with the public. Excellent communication skills, verbal and written. Two years' experience working with computers, including Microsoft Word and Excel, email and internet. Proof of valid driver's license and current/adequate vehicle insurance coverage. Ability to analyze data and reports. Tri-Valley is a non-profit community action agency.
