Job summary
The primary purpose of your position is to provide routine daily nursing care and services to assigned residents.You will be responsible for recording entries, reporting changes in resident's condition, and assisting with admissions, transfers, and discharges.Your duties will also include personal nursing care functions, special nursing care functions, food service functions, and safety and sanitation responsibilities.

Responsibilities
• Use wristband or photo card file to identify residents before administering treatments, serving meals, etc.• Record all entries on flow sheets, notes, charts, and computer programs.• Report changes in resident's condition to Nurse Supervisor, LPN, or RN.• Assist with admissions, transfers, and discharges.• Escort residents to their rooms and introduce them to roommates and other residents.• Make residents comfortable and assist with personal care tasks.• Assist residents with dressing, undressing, hair care, and nail care.• Keep residents dry, change bed linens, and make beds.• Assist residents with bowel and bladder functions, maintain intake and output records.• Assist with transporting residents, lifting, turning, and moving them as necessary.• Perform restorative and rehabilitative procedures, change dressings and provide eye and ear care.• Assist with daily dental and mouth care, bathing, and additional personal care tasks.• Observe and report pressure areas, perform daily catheter and perineal care.• Provide range of motion exercises, turn bedfast residents, perform diabetic urine testing.• Assist with feeding, record food and fluid intake, and provide after-meal care.• Ensure safety and sanitation by following established procedures and protocols.• Attend training programs and participate in Facility in-service training.
Requirements
• High school diploma or equivalent.• Licensed as a Certified Nursing Assistant/Geriatric Nursing Assistant in the state.• One year of experience as a Certified Nursing Assistant/Geriatric Nursing Assistant.• Ability to read, write, speak, and understand English.• Knowledge and skills to provide age-appropriate care.• Ability to make independent decisions and deal tactfully with various individuals.• Willingness to seek out new methods and principles for nursing practices.• Physical abilities to move intermittently throughout the workday, see, hear, and meet general health requirements set by the Facility.• Ability to lift between 5 to 45 pounds periodically.
