About the Role:
As a Manager of Health Navigators, you'll play a pivotal role in our mission to enhance healthcare delivery in rural areas. You'll lead a team of dedicated Health Navigators, fostering meaningful connections with our partner clinics and driving positive change within our organization.
Responsibilities:
Lead and manage a team of Health Navigators, providing guidance and support to ensure their success.
Cultivate strong relationships with partner clinic administrators, providers, and staff, addressing inquiries and fostering collaboration.
Collaborate with Market and Regional Leaders to recruit, train, and oversee Health Navigators, empowering them to excel in their roles.
Develop and implement healthcare education and professional development programs for Health Navigators.
Establish effective communication channels between Health Navigators and the central office, promoting transparency and efficiency.
Analyze operational and clinical data to identify areas for improvement and implement strategic solutions.
Offer support and guidance to Health Navigators in navigating operational and clinical challenges.
Contribute to continuous process improvement initiatives to optimize our operational outcomes.
Conduct regular visits to primary care partner clinic locations, providing hands-on support and engaging with key stakeholders.
Requirements:

Bachelor's degree required; clinical license (Nursing or Social Work) strongly preferred.
