Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits Manage team of individuals who provide patient care Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's needs Manage emergency situations in a prompt, precise, and professional manner. Participate in the agency's performance improvement program. Provide nursing care in accordance with applicable regulations and Company policy. Prepare and maintain accurate and complete clinical and progress notes on a timely basis. Other duties as needed to ensure the health and safety of patients. Active RN state license Valid drivers' license Five or more years of applicable nursing experience Minimum of 1 year supervisory/leadership experience Home Health Experience preferred. Knowledge of home health, private duty nursing, Medicare, Medicaid, Ohio Home Care Waiver required. *Eligibility for certain benefits may depend on employment status
