The Office of Student Health and Wellness (OSHW) aims to remove health-related barriers to learning such that students may succeed in college, career, and life.
Job Summary:
Under the direction of the Program Manager, the Children, and Family Benefits Unit (CFBU) Coordinator is responsible for conducting educational outreach to CPS families, staff, and community members to increase enrollment, engagement, and utilization (EEU) of Social Services including but not limited to Medicaid, SNAP, and TANF. They provide frontline support and coach schools in developing systems to address the health and wellness needs of students and the school community including connecting students with appropriate internal and external health services.

The Children and Family Benefits Unit Coordinator will be held accountable for the following responsibilities:
Complete a minimum of 25 household SNAP, Medicaid, CHIP, and TANF applications per month.

Conduct virtual and in-person school technical assistance and community-based outreach to all schools within an assigned network to educate, increase, and support:

Navigation of social service resources, education, counseling, and advocacy.

Providing health care knowledge, Medicaid and SNAP program enrollment, and information on public benefits rights, eligibility, and renewal for CPS families.

Work with all schools within an assigned network to ensure all schools have a Medicaid Designee

Conduct technical assistance at schools through a variety of methods including but not limited to presenting at school-led parent meetings, hosting parent workshops and outreach events focused on enrollment and utilization of public benefits, etc.

Provide Health Risk Screening referral assistance to a minimum of 20 households per month.

Serve as the Children and Family Benefits Unit network expert and first point of contact for all health-related issues and concerns raised by schools and communities within an assigned network to:

Increase social service Enrollment, Engagement, and Utilization by providing case management, health education, and outreach.

Increase Healthy CPS school alignment per CPS 5-Year vision and goals.

Provide implementation support and technical assistance to schools trained in key OSHW strategies.

Cultivate and maintain partnerships with community-based organizations and state agencies at the local level such as the Illinois Department of Healthcare and Family Services and the Illinois Department of Human Services.

Work closely with Senior CFBU Coordinators and CFBU Team Leads to develop and operationalize sustainable processes to support functional Enrollment, Engagement, and Utilization of Care improvements for the OSHW.

Responds to and completes tasks relevant to OSHW business processes per the request of management, such as covering hotline response during periods of high call volume, COVID-19 support, or as requested by leadership.

Support and lead district-wide health and wellness-related professional learning.

Attends staff meetings and professional development.

Other duties as assigned.

In order to be successful and achieve the above responsibilities, the Children and Family Benefits Unit Coordinator must possess the following qualifications:
Education Required:
High school diploma or GED equivalent required

Bachelor's degree from an accredited college or university preferred

Experience Required:
Minimum 2 years of experience working with healthcare education, in a school setting, or with public benefits programs.

Background in All Kids, Medicaid, and the SNAP programs is preferred.

Experience working consultatively and collaboratively with a team of professionals.

Experience managing multiple tasks in a fast-paced environment.

Knowledge, Skills, and Abilities:
Multilingual preferred (English, Spanish, Chinese, Hindi, Polish, Arabic, American Sign Language, etc.)

Knowledge of Chicago's communities and the social determinants of health.

Exceptional writing, verbal, and interpersonal skills, including public speaking.

Able to work in a fast-paced environment independently, function effectively, and collaboratively work in a team environment with other CPS departments, with a high level of personal investment in contributing to the priorities of the organization as a whole.

Ability to work with minimum supervision.

Excellent organizational skills and ability to effectively multi-task.

Possess integrity and tact when dealing with confidential information and sensitive situations.

Must be proficient with the use of computers and software, including but not limited to: Google Suite, Microsoft Excel, Word, and PowerPoint.

Must possess a valid driver's license, with reliable transportation and travel to sites across the City of Chicago.

Must be able to work outside of traditional office hours, including evenings and weekends as needed.

Conditions of Employment:
As a condition of employment with the Chicago Public Schools (CPS), employees are required to:
Establish/Maintain Chicago Residency

- Employees are required to live within the geographic boundaries of the City of Chicago within six months of their CPS hire date and maintain residency throughout their employment with the district. The Chicago residency requirement does not apply to temporary/part-time positions, however, all CPS employees must be residents of Illinois.

Be Fully Vaccinated Against COVID-19

- Unless approved for a medical or religious exemption, all employees are required to be up-to-date on COVID-19 vaccinations, including boosters, and to submit proof of vaccination to the district within 30 days of hire. "Up-to-date" on vaccination is defined as being at least two weeks past all primary vaccine doses and any applicable boosters.
