Responsibilities
• Provide spiritual care support to patients, family members, and caregivers• Conduct spiritual assessments of patients and family members• Assist with bereavement follow-up care• Participate in care conferences and meetings• Provide support to hospice team members• Develop relationships with spiritual leaders• Participate in quality improvement programs• Participate in on-call rotation
Requirements
• Nonjudgmental and inclusive presence• Spiritual Care experience• Knowledge and commitment to hospice philosophy of care• Awareness of end-of-life practices in different faith traditions• Hospice care experience preferred• Excellent communication skills• Ability to work independently and as a team member• Ability to use electronic medical resources• Meet federal and state requirements regarding background checks• Proof of valid driver's license and reliable transportation
