Job Description:
The Nurse Care Manager works collaboratively with physicians and other members of the health care team to improve the health of patients with chronic conditions or complex needs. This position educates patients and families to help them manage their health care needs. The incumbent facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.
Posting Specifics
+ Shift Details: 8:00am - 4:30pm Monday through Friday with a small weekend commitment of two weekends every six weeks.
Scope
As a Case Manager RN you need to know how to:
+ Coordinate the care and services for patients identified as needing assistance or meeting Care Management criteria and develop a plan to meet those needs.
+ Work with patient and family/caregivers to determine specific goals and actions based on assessment.
+ Assess the patient's prior level of functioning, access to and/or use of community resources and available support systems.
+ Identify appropriate admission and continued stay issues. Help to identify strategies for reducing length of stay and appropriate utilization of services. Utilize criteria including clinical pathway data to monitor efficiency and evaluate outcomes.
+ Attend, facilitate and participate in rounds and case conferences. Collaborate with Physicians and other members of the health care team on the patient's behalf. Coordinate discharge planning.
Requirements :
+ Graduate of an accredited school of nursing
+ Current Colorado RN license or compact license with the ability to obtain Colorado RN license
+ Minimum of three (3) years of experience as a Registered Nurse in an acute care setting
Preferred :
+ At least five (5) years of nursing experience
+ Prior knowledge of managed care / utilization review
Physical Requirements:
+ Interact with others by effectively communicating, both orally and in writing.
+ Operate computers and other office equipment requiring the ability to move fingers and hands.
+ See and read computer monitors and documents.
+ Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.
+ May require lifting and transporting objects and office supplies, bending, kneeling and reaching
