Position Summary:
Provide clinical support and education to elective surgical patients for the assigned program, and identify candidates for surgery. The program includes meeting with patients in clinic, working with surgeons and other staff at multiple health sciences campuses, assisting patients and family members with the surgery and recovery process; triage; preoperative screening and counseling sessions; clinical pathways and proactive coordinated planning for discharge and follow up care. The program is geared toward increasing patient satisfaction and patient outcomes, targeting zero surgical infection rates, evaluating patients who need inpatient rehab after discharge, reducing length of stay, and increasing inpatient/outpatient volumes. Devotes a majority of work time to supervisory duties and regularly directs the work of two or more other employees. The RN Program Coordinator collaborates with physicians and interdisciplinary team members in the internal and external operations to meet patient and family needs, develop guidelines, provide education, implement necessary research, monitor patient outcomes, suggest and implement successful initiatives to improve quality and grow volume in the program while marketing the product line. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include adolescent, adult and geriatric age groups.

Detailed responsibilities:
* PROGRAMS - Participate in programs and committees related to the specialty or as directed
* PATIENT CARE - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they do not understand or may "Not seem right"
* MISCELLANEOUS - Perform miscellaneous job-related duties as assigned
* EVALUATION - Administers or makes recommendations regarding performance evaluations, promotions, administration of personnel policies with staff, hiring or discipline
* QUALITY - Participate in quality improvement processes
* PATIENT SAFETY - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
* POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs
* PATIENT SAFETY - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
* QUALITY - Proactively coordinate and communicate with various departments (both external and internal) to coordinate services, resolve operation issues, and improve quality of patient care
* ASSIST - Assist physician and coordinate diagnostic procedures for pre-surgery and post-surgery needs
* ASSIST - Assist in specialty clinic; support triage for selection of surgical candidates
* DOCUMENTATION - Provide and document nursing care, and ensure that physician's orders are carried out
* PATIENT CARE - Serve as liaison between patient/family and various members of the health care team, including internal and external members, and multiple system campuses
* INTERVENTIONS - Counsel and teach patients and families regarding medical conditions, diagnostic procedures, physical therapy, medications, pain control, and other interventions
* REVIEW - Continuously reviews processes for operational improvement and efficiencies
* TRACK - Track progress of the program and identify areas that require attention
* STATS - Support statistical data collection and report to leadership and designated committees
* PATIENT SAFETY - Report potential and actual patient safety, concerns, medical errors and or near misses in a timely manner thru the chain of command and document within the PSN system
* PATIENT CARE - Establish a compassionate environment by providing emotional psychological and spiritual support to patient, friends and families
* PATIENT CARE - Promote patient's independence by establishing patient care goals; teaching patient, friends, identified ley caregiver and family to understand condition, medications, and self-care skills; answering questions
* TEAMWORK - Maintain a cooperative relationship among health care teams and resolve patient problems and needs by utilizing multidisciplinary team strategies
* COMPLIANCE - Active participation in hospital strategic initiatives such as quality improvement and monitoring TJC compliance
* PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes

Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
