Job Responsibilities:· Receive and Review Discovery form and documentation provided by Sales and Marketing Department via assessment request email. After reviewing documentation admission assessment must be scheduled and completed asap.
· Once assessment is complete findings are to be reviewed with Director of Nursing for determination of admission. All denials must be reviewed with Regional Admissions Coordinator before they are processed.
· The following assessments are to be completed and maintained in office and resident file:
1. General Service Evaluation (GSE) 1. This is a 9-page assessment packet that is updated upon admission, every 6 months, and as needed. i. Wellness baseline evaluation (2 pages) ii. Pain assessment (1 page) (if applicable) iii. Mobility management planning tool (1 page) iv. Self-med assessment (1 page) (if applicable) v. Elopement risk assessment form (1 page) (if applicable) vi. Mini mental (1 page) vii. Level of Care assessment*** (2 pages) - Any time LOC is updated AL coordinator and Business Office Manager must receive a copy.
2. General Service Plan (GSP)
 · Completed in visual. Must be printed and signed when updated. Placed in GSP binder and resident file. 3. Initiate Health Service Plan (HCP) (if applicable for Hospice, Homecare, PT/OT/ST)
· Communicate all clinical and ADL findings to DON and AL coordinator.
· Assessments are to be completed every 6 months and with a change in resident condition.
· Responsible to follow up with families/POA/Resident with all changes in level of care.
· Responsible to notify billing with any changes in care levels for correct billing status (increase or decrease in care levels).
· All assessments should be kept by Nurse Liaison in binders; GSE binder, GSP binder, and HCP binder. All three binders must be brought to LOC meeting weekly.
· RN must review and sign each assessment completed.
· Will maintain documents in an organized manner.
· Will document communication with family/POA, MD and Resident
· Other duties as assigned.
