SUMMARY
The facility-based APP role is designed to deliver direct patient care services at the Provider level within the Post-Acute Care setting.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Embracing our companys philosophy, youll provide care that enhances each patients clinical status and quality of life.
Conducting Annual Wellness Visits for long-term Nursing Facility residents and potentially performing Annual History and Physicals as requested by Facilities.
Administering clinical interventional and episodic direct care visits, along with appropriate re-evaluation of care visits.
Offering ongoing evaluation and treatment for chronic, co-morbid illnesses as deemed medically appropriate.
Evaluating and treating residents for changes in condition.
Reviewing medications, orders, and diagnostic tests, and responding to clinical abnormalities as allowed and/or requested by collaborating physicians.
Coordinating care with collaborating physicians for comprehensive medical management.
Assisting with facility-wide illnesses, leveraging appropriate collaborations.
Coordinating care with other disciplines like Therapists, Specialists, and Social Workers.
Maintaining communication with families and attending family conferences whenever possible.
Undertaking other duties as assigned.
QUALIFICATIONS
Hold a valid nurse practitioner or physician assistant license in the state of operations, along with current board certification in your field.
Possess a minimum of a Bachelors and Masters Degree in Nursing from accredited Colleges or Universities.
Have experience in nursing; some experience as a Nurse Practitioner/Provider is preferred but not mandatory, although demonstrated critical thinking skills are essential.
Possess knowledge of Nursing and clinical Nurse Practitioner critical thinking skills, applied to aged, chronically ill, and convalescent residents.
Preferably exhibit strong leadership, interpersonal, and problem-solving skills.
Have excellent written and oral communication skills.
Exhibit traits of punctuality, organizational skills, and the ability to function effectively within a team.
