Administrative Director of Care Management Location Houston, TX (Medical Center area) : Overview: Valence Consulting is working with an esteemed healthcare organization to seek a dedicated Director of Care Management. This role is pivotal in implementing and overseeing the case management program, ensuring optimal utilization of healthcare resources and enhancing quality of patient care.

Required Skills: Current and valid RN license in Texas or LMSW license. LCSW, CCM, ACM, or FAACM certification. Minimum five years in utilization management, case management, discharge planning, or similar. Knowledge of clinical care standards and payer requirements. Proficiency in business planning and targeted outcomes. Experience in managed care, inpatient, outpatient, and home health continuum. Background in performance improvement concepts. Strong communication skills and ability to work collaboratively. : Collaborate with Directors of Case Management to coordinate local level program aspects. Facilitate program growth in line with enterprise philosophy, responding to healthcare changes. Achieve clinical and financial outcomes through case management processes. Manage budget and resources effectively. Liaise between care partners for collaborative patient care. Lead a high-performance team, incorporating leadership principles and vision. Utilize data-driven strategies for case management performance improvement. Educate stakeholders on case management processes and evolve the program to extend beyond acute inpatient settings. Ensure compliance with policies and standards, focusing on quality and efficient service delivery. Other Qualifications: Bachelor's in Nursing (BSN) or Master's in Social Work (MSW), Master's preferred. Experience working in a large healthcare facility (850-900 beds) is a plus.
