WHITE MOUNTAIN APACHE BEHAVIORAL HEALTH SERVICES, INC.JOB DESCRIPTIONPOSITION:TRBHA CARE COORDINATORCLASSIFICATION CODE: EXEMPT FULL TIMEDEPARTMENT:CLINICALREPORTS TO:TRBHA CARE COORDINATORLEADSTATEMENT OF JOB:This Positionprovidescare coordinationservices to clients enrolled or eligible to be enrolled withWhite MountainApache Behavioral HealthServicesTribal Regional Behavioral Health Authority.TheTRBHA Care Coordinatorprovidesa link between the client, client support system, community, and otherhealthcare and social service professionals.TheTRBHA Care Coordinatoraddressesany barriers to care and provide referrals for needs such as food, housing, education, and mental health services. In addition, they conduct outreach toengage community residents, assist residents with health system navigation, and to improve care coordination.This position will alsofunction asan advocate for the client with other serviceproviders when appropriate to do so.DUTIES AND RESPONSIBILITIES:Managesa caseload of clients/potential clientswho would benefit frombehavioral healthservices.Documents contact withclientstoassess needs, maintain requiredpaperwork,and engage family members. in accordance withArizona Health Care Cost Containment System (AHCCCS)and ABHS Standards.Assistin the management ofthe high needs/high costs staffing and data transfer with AHCCCS.Assistand managesthedistributionofnotices of admit from AHCCCS.Abide by all applicable policies, procedures, and program standards of ABHS, White Mountain Apache Tribe, Indian Health Services, Arizona Health Care Cost Containment System (AHCCCS) and other pertinent regulatory authorities.Conduct outreach, facilitate access to healthcare services and provideeducationandsupportin accordance with theArizona Health Care Cost Containment System (AHCCCS)guidelines forcare coordination. Ability to demonstrateahigh degree of flexibility and competence in adjusting to clientelewith varying degrees of traumatic experiences and complex psychological presentations.Ability to address a variety of diversespiritual, emotional, and social needs in multiple environments of anoften-high-riskpopulation, using a trauma-informed care approach while maintaining a highdegreeof professionalism and cultural awareness.Facilitates engagement ofclientsand their families inappropriatehealthcareservices.Possess the ability to work with a wide range of clients including adultsand families.Collaborates with court/probation,tribal/community agencies,families,and other natural support systems to ensure client's needs are met. Develops a strong working relationship withcommunity partners.Providescare coordinationservices at the clinic, in the community, adult shelters,residential facilities,hospital settingsand in client's homes to ensure client obtains all benefits and services necessary.Participates inmulti-disciplinarymeetings toadvocate for client and community to coordinateconsistent and timely provision of services;attendsandparticipates in pertinent AHCCCS andIHS meetingsasassigned.Ability to facilitate referral process to assure continuity of care.Ability to maintain client records in accordance with HIPAA, AHCCCS, ABHS, Federal, State, Tribal, andlocal requirements.Maintain ethical policies and guidelines as outlined in accepted State ethics laws, rules,policies,and principles and as outlined in discipline-specific code of ethics.Provide a positive role model to clients and staff through leadership, self-discipline, and genuine care for the treatment and recovery of clients. All clients and staff members must be treated with dignity and respect.Participation in all required ABHS training.Reports/attends work in timely manner.Must be reliable and communicate directly with supervisor in all matters of attendance.Performs other duties as assigned by supervisor.MINIMUMQUALIFICATIONS:21 years of age and possession of ahigh school diploma or high school equivalency diplomaand18 credit hours of post-high school education in a field related to behavioral health completed no morethanfour years before the date the individual begins providing behavioral health services and two years of full time behavioral health work experience; or high school diploma or high school equivalency diploma andfour years of full time behavioral health work experience. Associates/Bachelor'sdegree in mental health/human services or related field preferred; experiencecollaboratingwith Native Americanspreferred.Excellent written and oralcommunication skills; abilityto speak the Apachelanguage preferred.Demonstrate critical thinking, problem-solving and organizational and time management skills with ability to work independently, with flexibilitywithin prescribeddeadlines.Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors.MS Office proficiency and the ability to correctly operate a copier and fax machine.Valid Arizona Driver's License andreliable transportation.Abilityto pass all pre-employment testing,including, butnot limited to,fingerprint clearance andbackground check.**NOTE: Preference in filling vacancies is given to qualified Indian candidates in accordancewith the Indian Preference Act (Title25, U.S. Code, Section 472 and 473).ESSENTIAL FUNCTIONSThe physical characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of thejob.Constant: standing.
