Responsibilities
• Provide competent nursing care and respond to telephonic requests• Conduct triage and symptom management for patients• Manage medication protocols and refills• Educate patients on chronic conditions• Collaborate with the care team and monitor care plans• Form relationships with patients and caregivers• Create a welcoming environment and delegate tasks• Participate in care team meetings• Deliver exceptional patient experience• Perform point of care testing and procedures
Requirements
• Active RN licensure in good standing• BLS certification• EMR experience• Ability to assess patients remotely• Strong communication and assessment skills• 2+ years of healthcare experience as an RN
