Position Overview:
We are seeking a skilled and empathetic Registered Nurse (RN) to join our hospice care team. The ideal candidate will be passionate about delivering high-quality care to patients in their homes or hospice facilities. As an RN Hospice Nurse, you will work closely with patients, families, and a multidisciplinary team to ensure comfort, manage symptoms, and provide emotional support during end-of-life care.
Responsibilities:
* Conduct initial assessments and develop individualized care plans in collaboration with the interdisciplinary team.
* Administer medications, manage symptoms, and provide skilled nursing care to maintain patient comfort.
* Educate patients and families on disease progression, treatment options, and end-of-life care.
* Regularly assess patient needs and coordinate care with physicians, social workers, chaplains, and other healthcare professionals.
* Provide emotional support and guidance to patients and families through compassionate communication and active listening.
* Maintain accurate and timely documentation of patient care and progress.
Qualifications:
* Active Registered Nurse (RN) license
* Minimum 2-4 years of nursing experience, preferably in hospice, palliative care, or a related field.
* Strong clinical skills with a focus on pain and symptom management.
* Excellent communication and interpersonal skills.
* Compassionate and empathetic approach towards patients and families facing end-of-life care.
* Ability to work effectively in a multidisciplinary team.
