You will collaborate with an interdisciplinary team to provide holistic care, offer counseling and support, and address psychosocial factors affecting patients' end-of-life experiences.ResponsibilitiesConduct comprehensive psychosocial assessments of patients and their families to identify their emotional, social, and cultural needs.Develop and implement care plans that address the psychosocial, emotional, and spiritual aspects of patient care.Provide counseling and emotional support to patients and families facing end-of-life challenges, grief, and loss.Collaborate closely with the hospice interdisciplinary team, including physicians, nurses, chaplains, and other professionals, to ensure coordinated and patient-centered care.Assist patients and families in accessing community resources, financial assistance, and additional support services as needed.Maintain accurate and timely documentation of all patient interactions and care plans.Participate in team meetings, case conferences, and in-service education to stay current on hospice regulations and best practices.Promote a compassionate and inclusive environment that respects the diverse backgrounds and beliefs of patients and their families.QualificationsMaster's degree in Social Work (MSW) from an accredited program.Current licensure or eligibility for licensure as a Licensed Clinical Social Worker (LCSW) in Colorado.Previous experience in hospice or palliative care is preferred.Excellent communication and interpersonal skills.Compassion, empathy, and a strong commitment to patient-centered care.Ability to work effectively in a multidisciplinary team.Strong problem-solving and advocacy skills.Proficiency in documentation and record keeping.Who we areOvation Hospice is a Regional Hospice provider in the Western United States.
