Full-time position with benefits!Purpose of the Clinical Care Coordinator: Assure that all aspects of the resident's personal care and service plan are implemented while assuring that the resident is treated with dignity and respect, assisting each resident to be as independent and self-sufficient as possible.Qualifications: Must be a Licensed Practical Nurse in the state of Michigan; Proof of training in TB testing; Proof of training and competency in medication administration; Leadership ability. Must be able to physically and mentally perform essential functions of this position. Ability to communicate effectively in English. Must be able to communicate with a variety of personality types. Able to effectively supervise and support staff in providing direct service to residents; communicate with family members, physicians, labs, pharmacy, and ancillary personnel. Must demonstrate compassion towards residents. Must be able to pass the state mandated criminal background screening and random drug testing.Essential Functions:Treat all residents with dignity and respectProvide excellent care and service to the residents of this living center in accordance with their individual written plans of care and serviceReview new order logs daily to ensure all new orders were processed completelyMonitor ALL lab orders to ensure they are properly scheduled and implemented.Follow up with results and appropriate communicationMonitor Coumadin logs for completionSpot check med-tech charting according to episodic charting logTrack and administer resident TB skin tests and B-12 injectionsPerform quarterly med pass reviews with all Med TechsAssist with Med Tech training for new hiresInvestigate all med errors and forward proper documentation to RSCPerform med cart audits at least monthly; more frequently as needed.Review pharmacy delivery logs and follow through as necessaryReview all notifications to physicians PRIOR to being faxedCheck med rooms, and med room refrigerators for cleanliness and outdated medicationsMonitor Licensed Health Care Professional's book and create list of residents that need to be seen and give to oncoming SupervisorReview monthly summaries, vital signs and weightsCommunicate with physician and/or licensed health care professionals regarding resident condition, medications, treatments, etc. as needed to assure residents health needs are metCompliance with and uphold all Resident Rights including confidentialityComply with and enforce infection control protocolsCompliance with and enforcement of all rules and regulations governing Homes for the AgedCompliance with and enforcement of the guidelines stated in the Employee HandbookAssure all Med Tech staff chart the delivery of medications to the residentsOn-Call duties minimum of one weekend/month for Resident Services CoordinatorParticipates in Manager on Duty rotation as directed by AdministratorParticipates in quarterly Infection Prevention and Control meetingsParticipates in weekly safety meetingsGeneral Responsibilities:Be a good will ambassador for this living centerComply with Life Safety policies and periodic "drills"Documentation as requiredParticipate in training opportunitiesReport safety and health hazards to the RSCComply with rules and regulations governing Homes for the Aged Work cooperatively with others on the same shift and across shiftsReport to work as scheduled respond to resident requests for assistance, including "call lights'Physical & Mental Requirements:Able to lift 50#Able to stand, bend, twist, turn, stoop, squat, sit, kneel, pull, push, and walk long distances on hard surfaces, for extended periods of time.Ability to assist residents in showers, toileting, transferring, and all aspects of personal care.Ability to communicate verbally and in writing with residents, co-workers, and supervisors.Ability to comprehend written and verbal instructions.Emotional stability to appropriately handle resident care needs, including chronic illness, challenging behaviors, dementia-related issues, and death of residentsBenefits Offered:401(k)Dental insuranceEmployee discountHealth insurancePaid time offReferral programVision insuranceFull-time positionHourly Range: $27.00-$30.00/hour#INDADS
