• Apply knowledge of community resources for patient discharge needs• Manage discharge planning arrangements delegated by Nurse Care Manager• Communicate effectively with patients, families, providers, and payor sources• Maintain knowledge of regulatory requirements for discharge planning• Be punctual and follow company policies and procedures• Interact in a positive and constructive manner• Prioritize and multitask
Requirements
• 1-2 years of discharge planning experience in acute care hospital or skilled nursing facility preferred• Bachelor's degree in Social Work or related field of study preferred• Licensed Vocational Nursing license preferred
