Our SUD Case Managers have a wide variety of job duties that they fulfill, in a supportive, caring, environment at our residential treatment location in Loretto, MN. Located just 9 miles west of Wayzata, we are situated on a stunning, 178 acres of restored prairie, forest and lakefront. We share a gravel entrance road with the YMCA Camp Ihduhapi, and staff have often said they can 'feel their blood pressure go down as soon as they turn onto our dirt road'. A serene, nature-focused environment provides our clients with peace and serenity hard to find elsewhere in their lives, and this translates to staff, as well. Activities our case managers have led for their clients include taking them out for a group session on our Pontoon boat, or teaching them to fish as part of a mindfulness session; from baking cookies or having a BBQ at the chalet, to helping clients teach each other about their culture, with an Indigenous meal and smudging around a campfire. These are just a few of the creative ways our case managers have utilized our space and location. The possibilities are endless!
Vinland's residential program has 61 clients and approximately 10 Case Managers. We also have 4 mental health professionals on staff conducting mental health groups and one-on-one meetings, which complements our case managers' work immensely. We also have a psychologist and psychiatrist on staff. Because we work with a more specialized population, case managers generally have smaller group sizes, between 8-10 clients, depending on census.
We have the usual case management duties of coordinating the involvement of interdisciplinary services in the formulation of a comprehensive treatment plan; providing regular group and individual counseling sessions to participants and their families as scheduled; documentation of significant events, contact with outside agencies, people or groups, individual sessions, treatment plans and regular entries for group attendance as required by Vinland standards; and completing discharge summaries in a timely manner.
There are support services such as participating in weekly staff meetings to review client status and clinical data; participation in a regular on-call schedule providing direction to staff in the event of emergencies, shift vacancies, and other problem areas; assisting with the intake process and the completion of intake screening for potential admission, and the coordination of the involvement of outside agencies in the formulation of treatment plans, discharge plans and regular referral contact updates.
Of course, there are other requirements Vinland abides by, including all of our case managers understanding and complying with the standard of ethics as established by the Minnesota Department of Health/Board of Behavioral Health and therapy, and participation as a member of Vinland's interdisciplinary team in a manner which promotes professionalism, client-centered services, and staff cooperation. EXPERIENCE desired:
Those with an LADC or ADC-T certification. BA/BS degree in a behavioral science discipline and satisfactory completion of at least 30 classroom hours of education in each of the following areas: Chemical Abuse and Dependency, Chemical Use Assessment, and Case Management and State of Minnesota Licensure. Knowledge in family systems counseling. Knowledge of persons with disabilities. Excellent oral and written communication skills. Excellent organizational skills.
