We define a Sr. Community Outreach Specialist as being a frontline public health worker who is a trusted member of, and/or has an unusually close understanding of, the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
In this job, the Sr. Community Outreach Specialist will be responsible for helping patients navigate and access community resources, various social services, and the healthcare system. Sr. Community Outreach Specialist will be expected to build and maintain relationships with study participants, including reaching out to them on a regular basis via telephone, at the clinic site, or at participants' homes or agreed-upon places in the community. This will occur during specific follow-up visits and on an as-needed basis. They will be provided with appropriate training and oversight. Since this study is geared towards improving blood pressure control, we want a Sr. Community Outreach Specialist who is flexible, organized, and patient centered. The position will require frequent travel to Washington, D.C.
Specific Duties & Responsibilities Provide education on how to manage BP (Blood Pressure) through self-monitoring and healthy behaviors, including diet and exercise. Reinforce positive BP self-management behaviors through follow-up encounters. Provide training and reinforcement on the use of home blood pressure cuff, and a mobile health app. Link participants to existing clinical and administrative services. Link participants with community resources to address health-related social needs. Assist patients with the use of the mobile health application (Sphygmo) Oversee data management activities; document interactions with patients including information needed for research purposes. Any other duties at the discretion of the manager or PI of the project Maintain confidentiality of all clients in accordance with employers' procedures Respond to inquiries and requests for assistance from clients. Oversees & performs constructions of database, datasets, variables & data cleaning. Develop database for projects. Coordinate activities and events with other community outreach specialist on the team and also with multiple community-based organizations. Attend outreach events and oversees data collection and monitoring process. Mentor and guide community outreach specialist to improve the healthcare and quality of care for persons living with high blood pressure. Provide general information regarding high blood pressure to community members. Provide information on providers for high blood pressure treatment or schedule required intake and follow-up appointments and make appointment reminder calls. Provide information on and complete applications for assistance programs. Participate in health promotion and health education outreach activities. Participate in other research and/or practice-based initiatives as workload allows. File, retrieve, and maintain client records. Perform all work activities per established protocols and procedures. Attend meetings and trainings. Special Knowledge, Skills, & Abilities Attention to detail. Ability to deal with frequent changes, delays, and/or unexpected events. Ability to establish positive, supportive, and trusting relationships with and among patients, their family members, and colleagues. Ability to work collaboratively and effectively within a team. Having empathy and compassion for those struggling with their health and issues in their lives Reliable and dependable Strong organizational and effective time management skills Willingness to advocate for patients in the healthcare setting. Willingness to learn new things (such as new computer programs, i.e., REDCap; or new skills) Valid Maryland, Virginia, Delaware, or Washington, D.C driver's license and access to an insured automobile that lists employees on the insurance policy. Familiarity with Microsoft Office applications (i.e., Word, Excel, Outlook) Strong organizational skills with ability to work independently and prioritize multiple tasks. Must be willing to travel throughout - Washington, D.C. Ability to work with populations across ages, sexual orientations, socio-economic circumstances. Minimum Qualifications Bachelor's Degree. Some related work experience. Additional experience may substitute for required education, to the extent permitted by the JHU equivalency formula. Preferred Qualifications Spanish Bi-Lingual. Strong communication in Spanish is preferred but not required. Knowledge of medical terminology a plus. Classified Title : Sr. Community Outreach Specialist

Starting Salary Range : $17.00 - $30.00 HRLY ($45,825 targeted; Commensurate with experience)
