Responsibilities
• Provide quality case management and psycho-therapeutic services to adult patients and their families• Participate as a member of the treatment team in developing treatment plans• Develop and coordinate an individualized discharge plan for the patient• Document all clinical services provided as it pertains to social services• Support an organizational culture for Service Excellence and practices the Service Excellence standards to all customer groups
Requirements
• Master's prepared clinician from an accredited school or college of social work• Must have South Carolina LMSW or LPC within one year of hire date• Prefer a minimum of two (2) years' clinical experience in behavioral health• Current CPI training prior or upon hire and maintain certifications throughout employment
