Position Summary: Coordinates the care of the adult cardiac valve patient. Maintains process to meet physical, educational and psychosocial needs of the patients and their families. Utilizes education, practice expertise , nursing skills, leadership and quality improvement procedures to provide seamless care for patients with valve disease. Provides support to nursing staff by coordinating the care process for the patients admitted to the hospital and seen in the outpatient clinic. Enhances patient care by facilitating communication between physicians, hospital staff, support services, and other community services. Develops a comprehensive treatment plan for patients with valvular heart disease. Provides education to meet the needs of the patient and their family. Provides accurate and timely data tracking. Communicates the Valve Clinic Program to Primary Care Physicians. Serves as valvular heart care resource to the members of the healthcare team. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke's mission and values.
Education, Experience, & Licensing Requirements: Education: Nursing Degree
Experience: P revious inpatient acute care of cardiac patients experience
Licensure: Current licensure as a Registered Nurse in the state of Missouri
