Learn more at northernnevadahealth.com This RN Case Manager opportuninty is per diem and offers a convenient flexible schedule.  This dynamic indibidual will be responsible for the implementation of the case management process, which is assessment, planning, collaboration and intervention, monitoring and evaluation.  Through this process, the Case Manager facilitates the patient's care from admission to discharge.  The Case Manager reviews care for level of care criteria, quality and efficacy of care and treatment, core measure interventions, and other criteria as requested.  Collaborates with all members of the healthcare team to facilitate discharge planning including providing patient and family education specific to disease management and follow up care.  Coordinates level of care referrals and planning with the social worker.
OR the Social Worker will provide services to patients in the acute hospital setting. May also provide social services to patients in the sub-acute geropsychiatric unit as needed. Collaborates with the RN Case Manager in providing discharge planning services to patients.  Is a resource for patients who require post acute care services, and psychosocial support during their stay in the hospital.
Job Duties/Responsibilities:  Case Manager: Performs the care management process to assess, plan and facilitate discharge planning on all patents. Documents initial assessment within 48 hours of admission. Demonstrates competency in use of Interqual criteria to determine appropriateness and level of care of admitted and continued stay patients.  Provides ongoing assessment and re-evaluation and documents appropriately. Documents according to current standards and practice including discharge planning needs, criteria for admission and continued stay review. Social Worker: Demonstrates ability, and is responsible to perform psychosocial assessments on Medicare, Medicaid and any high-risk patients to determine social service and possible discharge planning needs.  Documentation of assessments and interventions is timely and meets current practice standards and policies. Maintains current knowledge of resources available within the community; maintains supply of resource materials to be distributed to patients when needed.  Is able to obtain other resources as needed. Communicates with family members and caretakers regarding the needs of the patient and anticipated plans.  Involves patient and family in the plan of care. Qualifications Case Manager : 
Graduate from an accredited school of nursing, associate's degree required, baccalaureate degree preferred. Current Nevada Registered Nurse License. Five (5) or more years of previous experience in all aspects of nursing and two (2) or more years of UR or CM experience preferred Current BLS certification Social Worker:  Masters Social Work (MSW) preferred, BSW required. Current state Social Worker licensure. 3 or more years of previous experience in all aspects of social service and discharge planning preferably in an acute care setting. EEO Statement
