RN Case Manager :
Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each participant's unique needs

Home Health RN :

RN Job Requirements :
Graduation from an accredited nursing program and a minimum of one (1) year of experience as an RN
Current and unencumbered RN license in California
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Prior experience in senior healthcare or PACE preferred, but not required

Salary/Wage base range for this role is $47.74 - $63.02 hourly + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and role.
