The Unaccompanied Children (UC) Transitional Foster Care (TFC) Program Supervisor is primarily
responsible for overseeing case support and case manager staff to ensure adherence with job
responsibilities, agency expectations, and provide a positive work environment, while effectively
maintaining a client caseload. This position is responsible for coordinating case management, safe and
timely release services, training new case managers, and supervising the work of other case managers. Conducts weekly supervision meetings with direct support staff to ensure adherence to the expected job responsibilities, provide feedback, and lend support. Provides coverage support for case managers within the program, as needed. Facilitates training for case support and case manager staff related to case management services for client's educational, therapeutic, medical, and legal needs. Maintains and update the Transitional Foster Care (TFC) or Long Term Foster Care (LTFC) foster care documents and forms, as necessary to ensure continuous compliance. Creates and implements new staff training for case support and case management staff, as needed. Maintains a caseload in compliance with contract terms and agency expectations. Participates on client file reviews, as needed. Attends TFC/LTFC supervisors team meetings based on relevancy. Partner with TFC/LTFC supervisory team in assessing the fidelity of policy. Implementations of case management and case support teams on an on-going basis. Provides informal peer-to-peer mentorship to new case managers to include but not limited to providing encouragement, while also promoting positivity, accountability, and modeling professional demeanor. Oversees the case support and case manager training completion compliance in accordance with Catholic Charities West Michigan (CCWM), Department of Health and Human Services (DHHS), and the Office of Refugee Resettlement (ORR). Effectively promotes the Sanctuary model of care within case management and case support teams. Attends weekly meetings with supervisor to consult on cases, and to review case plan and direction Stay up-to-date on DHHS, Council on Accreditation (COA), United States Conference of Catholic Bishops (USCCB), and the Office of Refugee Resettlement (ORR) policies and procedures. Sets up and maintains the medical record for each program client. Ensures the initial medical screening, TB screening, immunization records, and medical recommendations are included in the medical record. Conducts frequent concurrent audits to ensure data entry and medical records are accurate and correct. Addresses specific medical conditions with medical providers and resources, while serving as a liaison with the provider and foster family. Assists foster parents and case managers in addressing any barriers that interfere with timely medical services for clients. Assists the program with communication medical needs, conditions, recommended treatment, and medical emergencies to Maintains accurate, up-to-ate information on clients in care while meeting ORR and CCWM's documentation and reporting requirements. Stays abreast of all agency, federal, and state regulatory requirements related to social services. Drives for Agency business. Performs other special assignments at supervisor's request. Ability to service a different group of clients, to include refugees or other minority families. Demonstrated leadership skills in alignment with agency and program values, while also exhibiting a team-player attitude. Ability to communicate clearly, effectively, and concisely, both orally and in writing. Ability to work independently and exercise good judgment. Ability to relate to diverse populations and cultures while communicating with clients and the public in a courteous and effective manner. Adheres to all agency and departmental safety procedures including reporting any unsafe practices, equipment and environment, and take an active role in correcting the unsafe practice, equipment or environmental through proper notification channels. Ability to work effectively on a multi-disciplinary treatment team. Ability to build rapport with clients to help them adjust to new life circumstances. Ability to provide excellent and compassionate customer service. Ability to work independently and exercise good judgement, and a high level of confidentiality. Computer and typing skills sufficient to perform essential job functions. Ability to relate to diverse populations and cultures while communicating with clients and the public in a courteous and effective manner. Drives for Agency business. Performs other special assignments at supervisor's request. Education: A master's degree in the behavioral sciences, human services or social services fields is preferred.
A bachelor's degree in human services or related field of study is required.
Experience: A minimum of three years of progressive employment experience that demonstrates supervisory and case management experience in child and family services is required.
Bilingual Spanish/English fluency strongly preferred.
Certificates, Licenses, Registrations: None required for this position.
Supervisory Responsibilities: This position does have supervisory responsibilities.
