The Population Health Pharmacy Technician supports the Fast Pace Pharmacy Team in an effort to enhance workflows for the clinical pharmacy services throughout the network, including medication therapy management (i.e. medication reviews and chronic disease management). This centrally located remote role provides patient identification, data analysis, chart review, patient outreach, and scheduling support for the Pharmacist completing clinical services within the organization. This role also provides telephonic patient care and support (i.e. pre-visit medication reconciliation, adherence monitoring, etc.) and supported data tracking, trending, and reporting. The Population Health Pharmacy Technician will work with minimum supervision and be comfortable working one on one with providers in areas identified for improvement.
This is a fully remote role with working hours of 8-5pm CST Monday-Friday.
What You Will Do: Performs medication reviews for patients as requested by provider or a member of the
population health team. Conducts chart review of patients with open pharmacy-related care gaps and triages patients for pharmacist review. Identifies gaps in diabetes and hypertension standards of care by using inclusion and exclusion criteria provided by pharmacist Conducts outreach to at-risk patients to discuss payer specific pharmacy options in efforts to increase adherence Supports providers and patients in payer formulary and tier navigation Evaluates patient referrals and takes appropriates steps within scope or escalates to appropriate population health team member Gathers data and generates reports to improve clinical pharmacy quality metrics as well as
pharmaceutical cost. Records medication information in the EHR and updates existing information as needed to
ensure accuracy. Performs outreach to patients identified from medication adherence-related reports, high-risk patient reports, or MTM eligible reports. Identifies situations for referral to clinical pharmacist (i.e. medication adverse event, ineffective medication, drug-drug interactions, lab test indicated). Interacts with all levels of medical staff to discuss relevant patient information as warranted. Prepares and maintains patient education materials/handouts. Outreaches to patients to schedule a televisit or office visit with clinical pharmacist. Reports issues and concerns to clinical pharmacist and/or manager in a timely manner. Assist patients in handling prescription related issues including calling pharmacies and health plans. Conducts chart reviews and patient interviews to identify patient specific reasons for poor
medication adherence and resolves identified problems or escalates to appropriate population health team member Collaborates with external vendors to ensure efficient and effective delivery of services. Works with the Manager of Ambulatory Pharmacy to maintain policies and procedures of Pharmacy programs in relevant practice sites. Participates in training and orientations of new employees as needed. Collaborates across all staff in the practice. Education: Must Have: C ompleted degree from a formal pharmacy technician training program Nice to Have: Bachelor's degree  Experience: Must have 1+ year of pharmacy technical practice Must have 1+ year of experience with patient outreach programs Sales related experience and/or prior work in an accountable care organization is a plus Current License or Certification: CT Board of Pharmacy Technician registration/license Certified Pharmacy Tech (CPhT) with PTCB High School Diploma or Its Equivalent
