Duties and Responsibilities: Applies advanced specialty knowledge, clinical skills, and expertise to assess patients and develop a list of actual and potential needs/diagnoses Responsible for gathering health history of patients through interview of patient and/or family member and physical examination Identifies risk factors through discrimination of normal and abnormal findings obtained through health history, physical examination, laboratory and/or radiological test results, and behavioral assessment Applies advanced knowledge, skills, and expertise in planning and implementation of patient's care. Assessment and management of wellness with consultation or referral as needed Performs individual, marital, couples, group and crisis intervention counseling to patients experiencing social and emotional difficulties Referral of unstable, chronic illness Counseling and education relating to specific health care needs and/or health promoting activities Collaboration with physicians and other health care professionals in the provision of care Must maintain professional licenses and professional competency through continuing education Must maintain patient documentation in a prompt and comprehensive manner.
