Description The Alternative Response Team (ART) is looking for a Staff Nurse to respond in real time with a Senior Social Worker to a variety of calls. The team will be embedded in the Brooklyn Park Police department and dispatched by the Hennepin County Sheriff's Office dispatch center. The ART team will respond to a variety of call types in which the 911 caller defines as urgent, and which the dispatcher and/or responding officer does not flag as potentially violent or involving weapons and locations. The team may respond with or in lieu of patrol officers to a call. The Registered Nurse will be responsible for completing a needs assessment and providing solution focused interventions in person, refer for follow-up and complete documentation.


Behavioral Health is an innovative and growing service area within Hennepin County's Human Services department. We provide a range of mental health and substance use services to residents including populations intersecting children, adults, and criminal justice. We have 230+ staff including social workers, psychologists, program managers, supervisors, planners, and others working collaboratively to meet our system goals of expanded access, upstream help, and person-centered services.

Location and hours:
This position is located out of the Brooklyn Park Police Department, 5400 85th Avenue North, Brooklyn Park, MN 55443 and will require travel throughout various cities in Hennepin County. Work hours are Monday through Friday 2:00 p.m. – 10:30 p.m. with the need for occasional flexibility.

In this position, you will: Develop collaborative relationships with law enforcement, emergency departments, human services, and other community stakeholders/partners. Complete health/nursing assessments, including checking vital signs, cognitive, psychosocial, risk, needs, spiritual, medical, dental, nutritional, chemical, and mental health status, to offer the most appropriate nursing, medical, and/or additional support services. Conduct initial medical and suicide risk assessments where a person resides, (e.g., in one's home and with people who are experiencing homelessness) at county facilities, and police departments (as appropriate). Deliver skilled independent and interdependent professional nursing care to individuals referred through urgent 911 behavioral health-related calls. Read, review, and understand medical records and share with other team members when available and as appropriate. Monitor medication, provide med education, including but not limited to, diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), birth control, HIV, Hepatitis C, sexually transmitted infections, the safety and importance of vaccinations, nutritional assessment, health assessment, and make necessary referrals for ongoing medical support. Document referrals received and case notes utilizing state and county systems. As needed transport individuals in county vehicle to community providers to further stabilize. Utilize solution focused interventions and together with the individual in crisis, create a personalized crisis/safety plan that is culturally informed, and person centered. Coordinate placement into a facility that provides a higher level of care when individual is assessed to be unsafe to remain in the community. Facilitate immediate service coordination with community providers and connect individuals to new and existing supports. Provide consultation, support and resources to police officers when responding to behavioral health related calls. Participate in community engagement events. Need to have: Graduated from an accredited school of nursing. Possession of a valid license, in good standing, as a Registered Nurse, or a permit to practice, issued by the State of Minnesota. CPR certification. A valid driver's license, the ability to obtain a Hennepin County driver permit and daily access to reliable transportation sufficient to transport clients, meet with clients, and/or perform authorized off-site work duties. Nice to have: Experience: Effectively interacting with people from diverse backgrounds including efforts towards disparity reduction. Providing home and community-based services with training and certification specific to assessment and consultation for substance use disorder, mental health care and pregnancy care. Working with individuals with complex medical needs, substance use disorders, serious and persistent mental illness, or developmental delays and the services and networks that serve them. Providing case management. Ability to: Function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgment. Work well in a group setting and be accountable to the team. About the department:
