LOCATION : Venice, CA

ABOUT THE ROLE: A member of the Housing team, the A Bridge Home (ABH) Case Manager will work at the ABH program in Venice (low-barrier, high tolerance) to provide case management services for SPY members experiencing homelessness. The ABH Case Manager is expected to provide support and services to residents, most of which have a variety of complex health, functional, and social needs, in order to help them transition back into the community. The ABH Case Manager will assist residents in breaking the cycle of homelessness by establishing and pursuing a permanent housing plan, and aiding residents in accessing and maintaining other supportive services in the community. The role will collaborate with other departments to ensure that residents have a successful onboarding, stay, and existence from the ABH project.
The ideal candidate will have a strong understanding of direct service to at-risk populations and trauma-informed care. Specific knowledge or experience related to transitional aged youth experiencing homelessness is a plus.
WHAT YOU'LL DO: Collaborate with Member Services Program Manager to build and create ABH Case Management Program that is inline with existing SPY Case Management Program Provide training and continual support with ABH Case Management team on direct services policies including but not limited to data input into Civicore (SPY's internal database) and Clarity/HMIS (CES regional database); administration of needed assessments; referrals to partner agencies; housing applications; problem solving philosophy; system navigation etc. Uphold SPY's cultural approach including philosophies of harm reduction, trauma informed care, healing centered, positive youth development, low barrier, housing first, crisis intervention and de-escalation Collect data for Case Management Program for monthly quarterly, yearly and donor reports as needed Supervise case managers and support workers along with other Senior Case manager supporting SPY members with varying needs Build individualized case plans for SPY members and support staff in developing and implementing individualized care plans around identified housing goals including increasing mainstream benefits; gathering necessary documents; connecting with education/employment services; and referring to health services as needed Meet with assigned residents a minimum of 1x/week to ensure progression towards housing and social goals Work within Youth Coordinated Entry System (YCES) to link young people to housing services and help them reunify with personal connections (family and friends) as appropriate Conduct and complete all resident intakes and administer Next Step Tools assessments and reassessments as needed Provide ongoing case management services that will support successful housing outcomes, decrease social isolation, increase member's sense of self-efficacy, reduce homelessness recidivism and deter evictions Collaborate with partner organizations to coordinate members' care and to ensure they receive the services and support they need to achieve and maintain physical and mental health, and housing stability Assist with housing applications, complete supportive and subsidized housing paperwork, and survey rental market for affordable housing Develop close working relationships with local service providers, outreach workers, case managers, landlords and collaborative agencies Provide ongoing information, referrals, linkages, and advocacy for members Provide all members with a home-like environment for the duration of their stay Complete and submit all required forms, case records, statistical reports and documentation to the ABH Program Manager within designated timelines Participate in developing new programs/services and collaborate with team members in implementing and creating work policies Collect data for programs for monthly quarterly, and yearly reports Transport youth as deemed necessary Utilize a harm-reduction, trauma-informed, and housing first approach Provide mediation and crisis intervention to youth as needed Participate in community and collaborative meetings as directed Other duties as assigned to support the goals of the organization. Requirements: WHAT YOU'LL NEED: Bachelor's Degree in Social Work, Psychology, Sociology or related field is preferred , but not required. 1-2 years of experience in case management, counseling or related experience is required . 2+ years experience providing direct services to high-risk youth including counseling and advocacy is required. A commitment to and passion for the mission, vision, values and culture of SPY. Knowledge of social service resources for youth experiencing and at-risk of homelessness. Cultural Responsiveness. A willingness to work collaboratively as part of the SPY team and the ability to work harmoniously as part of a diverse workforce A devotion to ensuring SPY's sites remain clean, beautiful, and a hopeful place for all who enter A self-starter who is prepared to drive innovative solutions for an emerging organization An ability to work in a fast paced environment that requires effective collaboration with a variety of organizations and individuals Knowledge of trauma-informed care, harm reduction and housing first models Must possess great communication and organizational skills Working knowledge of Microsoft Office (Word, Excel, PowerPoint, and Microsoft Outlook), and Google Drive (preferred) WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: Prolonged periods of sitting at a desk and working on a computer.

Physical effort/lifting, such as sedentary- up to 15-20 pounds at times.
