Lpn Care Coordinator

Responsibilities: Review patients' medical records to ensure that their plan of care is being followed and that patients' medical management needs are being met.
Your responsibilities will include chart audits and reviews, medical charting, maintaining a patient registry, and attending care team meetings.
Requirements: Valid LPN certificate/degree in FL
Basic healthcare background experience
Ability to work full-time
Detail oriented
Effective time management skills
