Mid-Columbia Center for Living (MCCFL) is a recovery-oriented, trauma-informed service provider for persons with serious mental illness, and addictions. MCCFL seeks to become consumer-involved, consumer-engaged and consumer-driven. The employee must embody principles of recovery, consumer-involvement and trauma-informed care as demonstrated in prior work experience, daily attitude and professional behaviors.



Introduction of the Classification :
The Family Care Coordinator (QMHA) is responsible for providing service planning and needs assessment to children and their families. This position assesses and monitors progress and also coordinates/refers resources as necessary. The Family Care Coordinator (QMHA) works in collaboration with community partners and the MCCFL Clinical Services Team to ensure that appropriate and effective services are provided to clients. With the high prevalence of trauma among individuals receiving mental health services, the Family Care Coordinator (QMHA) must possess demonstrated understanding of the effects of trauma on health, relationships, coping and other aspects of the lives of those we serve.


This position provides QMHA behavioral health services to clients age 2-17 and their families. This unit serves our most intensive levels of services with a focus on children and youth that are at risk of loss of placement from their homes, schools or community. Clients may present with a variety of issues including trauma histories, dysregulated behavior and/or emotions, suicidality, and difficult relationship dynamics. Interventions may include Collaborative Problem Solving, Cognitive Behavior Therapy, Dialectical Behavior therapy, Motivational Interviewing, and a variety of other modalities.



Essential Responsibilities of the General Classification : Assess the comprehensive needs of children and families including their current and potential strengths. Coordinate eligibility review team meetings and other service planning meetings for families, children, and youth. Facilitate the development of individual service plans and coordinate the identified services. Encompass families and youth as partners in the process of managing/coordinating care. Incorporate the strengths of families and youth, including the natural and social support networks on which families rely. Provide care coordination/management services to assigned clients. Services are provided in the home, virtual and/or a variety of community settings with consideration to client preferences for time and location. Actively participate in joint care planning and coordination with community partner service teams and MCCFL Clinical Services Team. Provide assistance with securing needed resources such as financial assistance, transportation, educational services, and treatment services as appropriate. Monitor and evaluate progress in appropriateness of services and achieving goals; provide timely feedback/updates to family regarding progress; encourage progress. Serve as an advocate for the child and family as appropriate. Utilize agency clinical software system to prepare current client records of treatment and reports as necessary and required. Responsible for the maintenance and timeliness of all records, progress notes, and other required documentation. Performs data tracking and reporting. Maintain current knowledge of administrative rules and statutes that govern the work. Support the goals of continuity and coordination of care across multiple services and systems over time. Position Specific Essential Responsibilities for the Intensive Outpatient Services and Supports (IOSS) for children (including accordance with OAR ******** ):
This position is for a Family Care Coordinator for the Intensive Outpatient Services and Supports (IOSS) IOSS services may be delivered to youths ages 2-17 at a clinic, facility, home, school, other provider or allied agency location, or other setting as identified by the child and family team. Triage and implement strategies for crisis and crisis stabilization with the family team or wraparound team Develop strategies designed to facilitate positive alternatives to challenging behavior and to assist the individual in developing adaptive and functional living skills; Document identification of strengths and needs and a proactive safety and crisis plan. Working knowledge of parenting skills/techniques and child development, and experience working with disadvantaged individuals and high-risk families, including work with sexual abuse, drug and alcohol abuse, domestic violence and trauma is also needed Participate in monthly wraparound team meeting with clients, families, Family Care Coordinator, and relevant community partners for integrated treatment planning and care coordination. Coordinate and implement in-home care needs with IOSS Clinician Facilitate psycho-educational groups. Develops and provides skill training, intensive skill groups, environmental interventions and case management services to individuals, families and groups. Provides behavioral health education and information to various interest groups. Assists and/or provides back-up support with screening, referral, and crisis services as needed and directed by supervisor. Actively participate in joint care planning and coordination with community partner service teams(schools, group homes, community detention facility, community medical hospital and other Department of Human Service programs) and MCCFL Clinical Services Team. Works with other staff and community representatives to initiate new programs, reviews, evaluates, and revises existing programs as necessary. Job Qualifications of the Classification : Incumbents in this position must be certified as a Qualified Mental Health Associate (QMHA). Any combination of experience and training equivalent to the following is qualifying: A Bachelor's degree in social work, psychology or related human services field and two (02) years' experience in human services; OR A high school diploma and five (05) years of equivalent training and work experience; or equivalent combination of education and experience. Experience working with families and children with severe emotional disturbances and/or early childhood education is preferred Bilingual fluency Spanish/English is required. Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR ******** to ******** . Must possess a valid Oregon or Washington Driver's license, maintain an acceptable driving record, and be insurable for client service purposes and for travel between business offices, meetings and/or trainings. Other Job Specific Knowledge, Skills and Abilities :
Knowledge: Knowledge of symptoms and challenges faced by children with severe emotional disturbances; knowledge of assessment and referral processes and procedures; knowledge of medication management; knowledge of MCCFL-based and other community-based services; knowledge of mental health and medical assessment, treatment and service terminology; knowledge of treatment/service planning; knowledge of medical records maintenance.


Skill: Skill in the use of clinical software systems; skill in the use of standard office software; skill in keyboarding.


Ability: Ability to understand and effectively apply the concepts and practices of a trauma-informed philosophy in performing the work. Ability to work within strong professional ethics and rules of confidentiality to provide effective services to clients. Ability to effectively work with clients to encourage and facilitate engagement in the treatment process; ability to maintain a strong service orientation; ability to read and understand a service plan and to effectively coordinate identified services; ability to write and maintain timely and cogent notes, reports and other professional documents; ability to organize and execute work effectively under the pressure of time-sensitive deadlines; ability to work with grace and good humor under pressure and to meet multiple deadlines; ability to work cooperatively with other departments and service providers; ability to communicate effectively, both orally and in writing; ability to effectively represent client and Agency interests to community partners and other stakeholders; ability to establish and maintain harmonious work relationships with co-workers at all levels of the organization, consultants, contractors, community partners, and consumers.



Leadership Received :
This position works under the supervisory leadership of a Clinical Supervisor. The designated supervision emphasizes independence of method and process while indicating accountability for results.



Leadership Exercised :
This position is not responsible for providing supervisory leadership to any other classification.



Physical Demands :
The Family Care Coordinator must have the ability to quickly assess and respond to concerns related to interpersonal and environmental safety as well as client care. While performing the essential duties of this position, the employee is regularly required to use office automation including computer and phone system that requires fine manipulation, grasping, typing and reaching. The employee may be required to sit or stand for several hours at a time, depending on environment and need. This position may occasionally be required to lift up to twenty (20) pounds, and to kneel, reach, bend or squat while in the office or in the community.



Work Environment :
Frequent travel in the tri-county region is required with work performed in various settings, including office environment, community settings and other agency stakeholder facilities. The office environment is typically with moderate stimuli and may often result in exposure to increased sensory experiences. Community settings, such as private residences, schools, police precincts, respite facilities, hospitals and other public spaces may pose additional risks or discomforts which require safety precautions. The Family Care Coordinator is expected to perform work responsibilities in the same settings in which employees are required to work.
