Neuroscience Group has a Full Time, onsite 40 hour per week position available for a team-oriented Concussion Program Coordinator. This individual can be an LPN, RN, or a Licensed Athletic Trainer. This position will typically work Monday - Friday,8 hours per day. As an integral member of the clinical services team, the Concussion Program Coordinator organizes and supports the program objectives by tracking and communicating patient data, monitoring progress and ensuring completion of pathway milestones. They will participate in patient care through various testing and treatment modalities including the Gyrostim. The coordinator will also serve as a liaison with community case managers, school and work officials in regard to activity release accommodations.

Ours is the only medical facility in the area that combines neurosurgery, neurology, orthopedic spine surgery, interventional and non-interventional pain management and physical therapy. RESPONSIBILITIES: Connecting with new concussion patients to complete questionnaires prior to visit. Documenting/entering data into Epic for new patient visits. Scheduling various appointments as ordered by provider Educating patients and their families on the Summit Concussion program pathway including what to expect, length of program and program requirements. Administering the Eye Box tests as ordered for concussion patients. Uploading Eye Box data for interpretation by the provider Administering the Right Eye test as ordered by the provider. Uploading the Right Eye data for interpretation by the provider. Support Gyrostim therapy when needed. Maintain concussion program data including tracking all new patients along the Summit program pathway, including completed standardized concussion data assessments (HIT-6, Concussion Grading Scale, CTSIB-M, FGA/DGI, NDI, DHI, RBANS) Support concussion staffing meetings in collaboration with PT/OT/SP/APNP and MD participation Maintain updated staffing data including: Updating staffing notes for each patient during the meeting, entering additional orders from discussions, and coordinate meetings with work comp case managers when requested Assist with brace fitting when needed Complete medical release/ disability/ school and work accommodation forms for concussion patients Monitor and maintain supplies and medication samples in the concussion clinic Weekly check/maintenance for Dr. Quick bag in concussion clinic Knowledgeable in the services provided within the concussion and physiatry clinic and services provided across the organization. Work collaboratively with a multi-disciplinary care team, patients, employers, schools, athletic departments, and third-party administrators (TPA's) to ensure timely progression along a continuum of care for patients with complex needs. Assist in minimizing fragmentation, redundancy, or delay of care by aligning resources to improve quality of care, reduce costs, improve outcomes, and reduce the time patients are impacted in their work, fitness aspirations, and quality of life due to a concussion. QUALIFICATIONS: Graduate of an accredited college or university with a Licensed Practical nursing certificate, RN, or LAT preferred A minimum of two years' experience in the field of neuroscience/concussion nursing preferred. Proficient with EPIC EHR Must be able to respond to both written and oral correspondence. Must be organized and be able to manage demanding workload with accuracy. BLS Certification must be current or obtained within the first six months of employment. BENEFITS: At Neuroscience Group we offer a very competitive salary.
