Community Care Worker

The role entails assessing needs and eligibility for Community Care for the Aged and Disabled (CCAD) services. This includes conducting home visits, interviewing older individuals or those with disabilities, documenting information, and developing service plans. Additionally, verifying data, explaining program benefits, referring individuals for services, and ensuring authorizations are recorded in automated systems are part of the responsibilities. Monitoring CCAD services to ensure they meet individual needs is also crucial.

High School Diploma or GED equivalent Experience with email and Microsoft Office products Willingness to travel approximately 70% of the time for home visits, trainings, and meetings Effective written communication skills Additional Information:
