The Job: We are currently recruiting for a dedicated and compassionate entry-level Case Manager . As a Case Manager, you will be responsible for providing support and assistance to individuals in need, helping them navigate through various challenges and connect with necessary resources. What You Will Be Doing As A Case Manager: Conduct comprehensive assessments to identify clients' needs and develop individualized care plans Provide counseling and guidance to clients, utilizing motivational interviewing techniques Collaborate with clients to set goals and develop strategies for achieving them Coordinate and facilitate referrals to community resources, such as social services, addiction counseling, Section 8 housing, behavioral health programs, and workforce development agencies Monitor clients' progress and provide ongoing support and advocacy Assist clients in crisis management situations, ensuring their safety and well-being Maintain accurate and up-to-date documentation of client interactions and progress Our Ideal Case Manager Has: Customer service orientation— proactively assist clients in a professional, friendly manner to create the best possible experience by building relationships, understanding quality standards for service, and utilizing customer needs assessments Social perceptiveness— be aware of other people's reactions and understand why they react the way they do. Maintain a positive, friendly composure even in difficult situations Discretion —act with prudence and circumspection, especially in the handling of confidential and private documents/data. Self-motivated— work autonomously to set and achieve targeted goals Communication— both verbal and written Ability to establish rapport with diverse populations Strong organizational skills and attention to detail Requirements To Be A Case Manager: Associate's Degree required; Bachelor's degree preferred in a field related to human services Previous experience in a patient support role is highly desirable Familiarity with medical terminology Katapult Network is an equal-opportunity employer and celebrates diversity in all of its forms.
