Responsibilities
• Assist in risk stratification of high-risk patients• Act as a patient advocate• Ensure highest standard of integrated healthcare• Make initial contact with patients assigned to ACC• Assess patient's medical, dental, behavioral, and social needs• Develop comprehensive care program for patients• Provide care management and support services to high-risk patients• Follow procedures for electronic health records system• Assist patients with applying and accessing assistance programs• Provide ongoing support to patients in the ACC
Requirements
• Bachelor's Degree in social work or health-related field preferred• Experience in care management, patient navigation, or social work preferred• Experience in community health setting or with underserved populations preferred• Computer knowledge and proficiency in Microsoft Office programs• Bilingual English/Spanish required
