Mobile Health Patient Transfer Center Specialist Job Summary The Specialist functions semi-independently, under the direction of the Transfer Center nurse, nurse supervisor as part of the integrated healthcare team. The Transfer Center Specialist will work to triage calls, coordinate transfers, transportation, dispatch Specialty Care Transport Unit SCTU units, complete pre-registration and bed placement along with all other aspects of transfer logistics under the supervision of the Transfer Center RN. Knowledge and necessary skills will be utilized to effectively meet the needs of transfer process. Must possess ability to establish and maintain effective working relationships with the public. Requires the ability to effectively plan, must be able to follow directions and to perform work according to department standards when no directions are given. Requires enthusiasm, initiative, and the ability to work independently/cooperatively with other personnel. Requires the ability to function effectively in a fast-paced environment with multiple demands and deadlines.
Essential Duties and Responsibilities Serves as the point of contact for referrals and obtains necessary information to initiate and facilitate the patient s transfer into, between or out of RWJBH Healthcare hospitals under RN supervision
Proficient in all Transfer Center workflows and protocols.
Works directly with staff at each client facility to ensure timely access, appropriate bed assignment, and accurate patient information documentation to assist with an appropriate destination plan of care at receiving facility. Receiving, documenting, and communicating time-sensitive clinical data from a multitude of sources.
Demonstrates the ability to triage and prioritize patient information under the direction of an RN and communicate an accurate clinical presentation of the patient for accepting physicians and nurses.
Coordinates complex ground and air transport logistics and internal response teams for transfers.
Proficient in Transfer Center database platform, telephony systems, and Office systems as required to support operations, ensuring accurate, timely data entry, protection of PHI.
Demonstrates knowledge of regulatory components to include but not limited to EMTALA, Ethics and Compliance, quality initiatives, and HIPAA; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives.
Effectively supports system, facility and department goals and objectives, mission, vision, and values through appropriate use of interpersonal skills and job performance. Support Transfer Center and RWJBH hospital's growth strategies.
Demonstrates well-defined organizational and time-management skills. Identifies, analyzes, organizes, and solves problems and issues in a timely, effective manner and collaborates with others to make sound, timely decisions.
Demonstrates commitment to and compliance with company s policies and procedures. Adheres to safety and infection control and prevention policies, procedures and guidelines.
Assists with non-patient related activities.
Assists in putting away supplies.
Assists in the containment of costs through the appropriate usage and conservation of supplies and equipment
Reports malfunctioning equipment and missing equipment.
Utilizes free time to the advantage of the department.
Maintains a safe and clean environment
Utilizes good communication skills.
Shows initiative in work tasks.
Maintains a favorable attitude toward job position and responsibilities
Ensures continuum of care by communicating with nurses and physicians.
Encourages and involves others in a tactful, cooperative manner by using appropriate interpersonal communication skills and teamwork.
Provides prompt, courteous, friendly service to all customers, patients, physicians, employees, volunteers, vendors, visitors, and patients' families
Accepts and follows instruction, guidance and or constructive criticism.

Appropriately answers the telephone, identifying self and department to caller.
Participates in the Quality Improvement process to improve patient outcomes.
Is aware of ongoing quality review issues and assists in collecting data when applicable.
Participates in the education processes which result from QI activities.
Complies with performance improvement recommendations.
Maintains RWJBH/department policies and procedures and QI program goals.
Complies with all policies and procedures that pertain to HIPAA including the minimum necessary requirements for this job position.
As a part of the requirements for this position, the employee has access to the entire medical record for the purpose of facilitating a patient transfer.
Limits the protected health information, PHI disclosed or requested to the amount reasonably necessary to achieve the purpose of the request. The disclosures or requests that occur on a routine basis include patient demographics, condition, notification of actual and potential victim of abuse or neglect, description of injuries for the purpose of a crime investigation, actual/potential for organ donation, and patient disposition updates.
Makes disclosures for the purpose of appropriate patient care and meeting of local and state requirements for reporting.
Criteria established to limit the PHI disclosures, requests to that which is reasonably necessary includes, communicating only the PHI that is required by law, upon request, and for correct and intended purpose of providing patient care and notification to local and state authorities and or regulatory agencies
Successfully completes department specific competency requirements and certifications for a Paramedic, EMT, Patient Care Technician and maintains a current license if applicable.
Successfully completes department specific mandatory requirements and maintains a current license if applicable.
Pursues professional growth and development including the latest trends by attending in-services, seminars, workshops and reading current literature.
Is self-motivated and demonstrates a need for professional growth.
Participate in EMTALA and Customer Service training and demonstrates proficiency.
Other related job tasks or responsibilities as assigned.
Knowledge, Skills, Abilities Must have a working knowledge of hospital operations, medical and nursing staff procedures, hospital, and community resources.
Must demonstrate critical thinking, conflict resolution, time management, and problem-solving skill.
Requires excellent communication skills and an ability to interact well across departments, facilities and organizations.
Excellent organizational and human relations are required to maintain good rapport and effective working relationships with internal and external customers.
Job Qualifications, Education, Training, Certification, Licensure High school diploma, GED or equivalent working knowledge. Bachelor s degree preferred.
The position requires skills and abilities typically gained with 2-3 years of experience in a healthcare office or hospital operations setting, and or educational background. Must have medical terminology gained by either college credits of experience in a health care setting, excellent verbal and written communication skills, organizational skills and critical thinking skills.
Must have the ability to work under tight time constraints and meet deadlines while working on a variety of projects under high demands.
Proficient technical ability to simultaneously manage multiple complex computer systems at a speed reflective of patient acuity needs. Must be proficient with commonly used office software and equipment including but not limited to all Microsoft applications, hospital EMR and bed placement software. Able to meet the agent productivity standards as set by Transfer Center Leadership
The following are preferred or obtain within 6 months of hire: American Heart Association, AHA. Basic Life Support BLS, HAZMAT Awareness, CBRNE, WMD Awareness, ICS 100, ICS 200, NIMS 700, ICS 800
Physical Requirements Prolonged periods of sitting at a desk and working on a computer
Must be able to lift up to 30 pounds at a time
