Job summary
The service coordinator (SC) is a service facilitator to residents with emphasis being placed on those who are frail and at-risk.They may provide limited or general case management, information and referral, plan educational programs and coordinate volunteer opportunities.The SC works with residents, property managers, and service providers to empower residents to age in place and remain as independent and self-reliant as possible.

Responsibilities
• Conduct intakes and assessments of residents to determine needs or services.• Maintain resident's files in accordance with HUD guidelines and grant compliance.• Assist and educate residents and families on necessary services to maintain independent living.• Plan, coordinate, and promote wellness educational activities for all residents.• Bring wellness/preventive health screenings and clinics into the project.• Assist residents in strengthening/building informal support networks among themselves and with family members.• Act as a liaison between community agencies, service providers, family, and residents.• Work as a team member with facility manager and other housing management staff.• Encourage and empower residents to meet their social, psychological, and physical needs.• Coordinate volunteer support programs with service organizations in the community.• Advocate and negotiate on behalf of residents for adequate, timely, and cost-effective provision of services.• Establish and maintain relationships with agencies and service providers.• Keep a current, updated directory of community services.• Attend all required training, meetings, and learning events.• Document contact with residents, providers, and families.• Meet reporting deadlines and present a dependable, reliable persona to residents and supervisors.• Perform other duties as assigned.
Requirements
• Bachelor's degree in Social Work, Gerontology, Psychology, or Counseling preferred.• 2-3 years of experience in social service delivery with senior citizens and non elderly disabled.• Some supervisory or management experience may be desirable.• Intermediate level knowledge of technology and ability to complete tasks online.• Working knowledge of community services with particular knowledge of those provided for the population in the facility.• Experience in service management or facilitation, organizing, problem-solving, and advocating.• Knowledge of the aging process, elder services, disability services, substance abuse, and mental health issues.• Knowledge of eligibility for federal and state entitlement programs.• Strong communication, writing, problem-solving, and organizational skills.
