This RN will be responsible for visiting patients in their home conduct patient visits to ensure that care plans are being followed and updated as necessary. Develop and maintain patient care plans according to agency policies and procedures. Communicate with patients, their families, and other healthcare professionals to coordinate care and ensure patient satisfaction. Conduct regular reviews of patient charts to ensure that all required documentation is accurate and up-to-date. Collaborate with other healthcare professionals to ensure that patients receive the highest level of care. Work with agency leadership to develop and implement quality improvement initiatives. Ensure that all patient information is kept confidential and in compliance with HIPAA regulations. Compensation:
