Title: LPN / LVN - SNF / LTC Responsibilities: Nurse care management: assess, plan, implement, coordinate, monitor, and evaluate the plan of care with the attending physicians. Provide well-documented, accurate and sensitive in-person advice, health education and self-care instruction. Support patients during times of crisis or transition to prevent avoidable hospital readmissions. Reach out to patients identified as high-risk. Qualifications: Valid LVN license. Ability to thrive in a fast-paced environment and adapt to change. Ability to work both independently and collaboratively within a team. Demonstrated clinical assessment skills.
