Your Specific Responsibilities: As an employee of the Florida Department of Health in Clay County, this position works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. This position reports to the Community Health Division Director.
Serves as the Community Health Needs Assessment and Community Health Improvement Plan (CHIP) Liaison for DOH-Clay. Plays a lead role in conducting the Community Health Needs Assessment and in the development of the Community Health Improvement Plan. Facilitates the community health action planning meetings, utilizing county vehicles, comprised of community leaders to identify and address critical health needs through community collaboration. Leads and supports health improvement projects and initiatives identified in implementation plans. Provides project management expertise to the CHIP team with regular meetings, documentation of activities, and progressive performance measures in accordance with the PHAB model. Works in partnership with internal and external stakeholders to develop and implement community health improvement strategies based on in-depth knowledge of evidence-based practice, previous successful efforts, input from community partners, and local community assets. Maintains scorecard of strategy activities and updates internal SharePoint website monthly.
Assures DOH-Clay is represented in the community through partnership building and collaboration. Identifies and develops strategic partnerships that will support further growth and improved health within our community. Attends local and regional meetings, conferences, and training that support our state health initiatives. Fosters effective team interaction both internal and external through open and honest communication, being receptive to feedback, and mentoring and developing others.
Serves as the Accreditation and Quality Improvement Liaison for DOH-Clay, coordinating all aspects of the projects to maintain accreditation status. Participates as a team member in the ongoing processes and improvements for maintaining accreditation across all domains and standards, serving as an internal consultant on processes that can be improved based on national standards. Coordinating with DOH-Clay programs to help support the facilitation of QI projects in all stages.
Organizes the Strategic Plan and associated activities to make certain that all partners, both internally and externally, are collaborating, communicating, and documenting their work as necessary. Provides updates on the Strategic Plan and Quality Improvement Plan at Leadership Council meetings as requested. Updates plans accordingly and assures timelines are met. Provides project management expertise and serves as a direct link of information sharing between all levels of staff to ensure the advancement of the Strategic Plan across areas.
Coordinates grants and other projects from Central Office related to Community Health. Writes status reports, annual reports, and talking points on strategic issues, including the development and dissemination of the DOH-Clay Annual Report. Analyzes data and identifies opportunities for strategic health improvement areas. Share insights and information across the organization to facilitate strategic thinking, innovation, and links to our community, and present findings at community engagements.
Participates in process improvement and problem-solving opportunities to enhance department efficiency and effectiveness. Presents findings at All Staff meetings, identifying training needs and arranging for quarterly trainings aimed at internal improvement across the board. Plans and facilitates Performance Management Council (PMC) meetings. Incumbent will be required to travel in the county and possibly outside of the county to attend meetings, conferences, trainings as required to complete their required job duties.
Other duties as assigned within the realm of a Government Operations Consultant II including participation in Special Needs Shelter functions.
Performs all the other duties as required. Required Knowledge, Skills, and Abilities: Ability to use Microsoft applications to analyze data and create reports. Knowledge of FDOH policies and procedures. Understand the Community Health Improvement framework and processes. Understand Community Health Assessment and Prioritization processes. Ability to lead, develop, and support group projects. Skilled at identifying leads from external agencies and engaging community partners to achieve a common goal. Ability to facilitate workgroup meetings with external and internal partners. Understand and ability to interpret data and effective use of findings. Qualifications: Minimum –
Must have at least an intermediate level of knowledge of Microsoft Office applications including Excel, and PowerPoint. Must have at least six months of verifiable experience working in public health. Must have a Minimum of a bachelor's degree from an accredited college or university in Public Health, Community Health, Health Education, Community Organizing, or Health Science or equivalent work experience. Preferred –
