Job Overview
The NICU Discharge Specialist, works closely with the NICU team to assure that all infants that meet the Indiana Perinatal Levels of Care criteria have an appointment at the Developmental Follow-up Clinic before NICU discharge. This individual will assure a smooth transition from the NICU to Developmental Follow-up Clinic and any other referral within or outside the organization.
Checks all NICU discharges for criteria to follow-up clinic.
Reviews infants that meet the criteria with Neonatologist.
Contacts parents to schedule discharge appointments for NICU infants that meet the Indiana Perinatal Levels of Care criteria for NICU follow-up appointments.
Works with parents to educate on follow-up services and intake paperwork.
Performs day to day operations including scheduling for Developmental Clinic.
Prepares intake chart and insurance information for follow-up clinic.
Obtains insurance authorizations for follow-up clinic.
Contacts affiliate hospitals to educate about the follow-up clinic and any referrals outside the clinic as needed.
Works with Neonatologists, social services and outside agencies for other solutions when patients do not meet criteria.
Requests additional records, calls parents and insurance when clarification is needed.
Coordinates care and appointments between NICU, Developmental clinic, Feeding Clinic and other referral sources.
Send, track and maintain referral orders.
Maintains tracking system and analytics defined by the Family Center Care Team to include but not limited to: wait times, cancelations, no shows and other items defined by Family Center Care Team.
Coordinates care with providers, clinic coordinators and follow-up team.
Education and Experience
The Discharge Specialist is preferred to be a graduate of a certified medical assistant program with 2+ years' experience within a NICU follow up, developmental, or feeding clinic and 3+ years' experience in maternal-infant care.
Current certification/license from one of the below:
Certified Medical Assistant- American Association of Medical Assistants
Registered Medical Assistant- American Medical Technologist
Certified Clinical Medical Assistant- National Health career Association
National Certified Medical Assistant- National Center for Competency Testing
Nationally Registered Certified Medical Assistant- National Association for Health Professionals
Registered Medical Assistant- American Allied Health
Certified Medical Assistant Clinical- American Medical Certification Association
