PRIMARY FUNCTION:
ChristianaCare is searching for a Community Educatorfor the Health Guide Program to support patients access insurance and financial assistance programs, along with connecting them to a variety of system and community-based resources which address the mediators of health outcomes. The team this position is joining is a close-knit work family that values teamwork and supporting each other where needed.
This position is located at Su Centro de Salud, with Primary Care at Kirkwood in Wilmington, DE.
Work Schedule
Mon-Fri, 8:00am-4:30pm
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Educates and connects uninsured and underinsured patients and community members to Medicaid, the Health Insurance Marketplace, Christiana Care financial assistance, Screening for Life/Health Care Connection program and other financial assistance programs.
Interviews patients and evaluates qualifications for programs mentioned above.
Screens patients for dental financial assistance, as needed.
Screens patients for Medical Legal Partnership eligibility and completes referrals, as needed.
Works in collaboration with Wilmington Health Center clinical teams to actively support patient care, as needed.
Maintains current knowledge of resources available in the community and actively seeks new resources.
Shares information with patients and CCHS staff about available patient assistance programs and community resources.
Track and document patient activity in RedCap and provide monthly statistics for database.
Works to create and continue strong relationships with CCHS staff and community partners.
Attends staff and department meetings and trainings as required.
Attends community events as required.
Adheres to Christiana Care policy with regard to release of information and patient confidentiality.
EDUCATION AND EXPERIENCE REQUIREMENTS:
High School Diploma required
Associates Degree in a Human Services-related field preferred
1-2 years of administrative experience required.
Bilingual required.
