About this role: The Kennedy-Donovan Center is seeking two Case Managers/Social Workers for the Foster Care and Family Services Division to assess and support individual child, youth, and family needs.
In this role you'll get to: Assess individual(s) and family strengths and  needs. Develop safe and helpful relationships with assigned individual(s). Participate in all team meetings such as: Development of Treatment Plans Ongoing supports related to Treatment Plan objectives and  goals. Individual Education Plans (IEPs) Foster Care Reviews Individual and case reviews Provide in-home case management services for families and  children. Advocate for children and their families as  needed. Provide supportive services to foster/biological families, as well as identified children/ youth. Collaborate with internal/external resources and community providers to ensure appropriate placement and  services. Monitor placements per DCF/EEC/Kennedy-Donovan Center  regulations. Comply with all Kennedy-Donovan Center, EEC and DCF documentation  requirements. Maintain compliance with all Kennedy-Donovan Center policies and  procedures. Transporting children/families as needed. We are looking for people who have: Bachelor's degree in Social Work or related field (Psychology, Human Services)  required. LSW or LSWA licensure (within 6 months of hire) . Driver's License and a reliable vehicle. Strong organizational, interpersonal and communication skills. Commitment to Family Systems focus. Computer competence in Microsoft Office Knowledge of Department of Children and Families system. Ability to work within multiple program models.
