Job Summary: The Mental Health Practitioner is responsible for the provision of rehabilitative services to adults living with serious and persistent mental illness as described in Department of Human Services Rule 78, the 1987 State of Minnesota Comprehensive Mental Health Act and provisions of the host county contract. These services are provided under the supervision of the Program Supervisor.
Major Job Responsibilities: Provide therapeutic habilitation/rehabilitation services to clients. Services are provided one to one basis for clients in need of support. Provide input to other service providers as approved by the client and required for measuring progress towards client goals.
Facilitate productive activities for clientele by developing and maintaining volunteer activities for clients and supporting clients in job seeking processes.
Facilitate psychosocial activities, independent living skill training, social/recreational Activities within homes and the drop in center. Provides crisis intervention and outreach to individual clients.
Assess/report the progress of client skills to community groups and within assigned meetings regarding CSP services. One on one assistance and support to families is also provided.
Provide case managers with factual and objective information to incorporate client needs/goals with services.
Enrich lives of clients in the community. Explain service options and requirements to clients and assist in arranging transportation when needed.
Function as a member of the Community Support Team by attending all staff meetings and staffing, providing updated reports on clients and input on client assessments.
Job Requirements: Education: High School Diploma
Job Related Experience: Must meet State of Minnesota requirements for a Mental Health Practitioner and have a minimum of 4,000 hours of supervised experience in the delivery of services to adults with mental illness, substance use disorder, emotional disturbance, traumatic brain injury, or developmental disabilities; OR Have a Bachelor's or Master's degree in behavioral sciences or related fields from an accredited college or university; OR Have at least 2,000 hours of supervised experience as above and receive clinical supervision until the 4,000 total hours have been obtained. Must have a valid driver's license and be able to travel multi-county area.
Job Related Experience: One (1) year of experience working with children in a daycare, day treatment, or school setting
